You and your family are invited to attend the Community Care Hospice Memorial Service on Sunday, May 7, 2017. 

The service will begin at 2:30pm at the Wilmington Church of God, located at 100 R Gordon Drive, which is off Nelson Avenue between Main St & SR 730.

The Memorial Service is a program of reflection, prayer, and remembrance of the special people served by Community Care Hospice over the past year.  This is an opportunity for family and friends to remember and honor their loved ones.  Our hope is that participation in the service will provide you and your family with an occasion to express your feelings and to remember your loved one in a special way.  To honor our loved ones alongside other families who are also working their own way down the path of grief recovery, can be a very healing experience. 

During the service we read aloud the names of the patients served and a member of each patient's family in attendance will be presented with a keepsake in memory of their loved one.  There will be a time for you to share a thought or memory if you care to.  A member of our staff would be glad to read a written remembrance, if you are not comfortable with sharing personally.  Additionally, the service will include a picture slide show of the patients served.  To include your loved one in the slideshow, please mail or email one picture only to our office with your RSVP by Wednesday May 3rd.  We will not be able to include pictures received after this date.

Community Care Hospice staff, including nurses, aides, social workers, chaplains and volunteers are invited to attend and participate.  Following the service, we will have fellowship and refreshments.  Please join us if you can.
